Clinical Competency Evaluation The deadline for the CDHO receiving completed applications is Wednesday, March 21 at 4:00 p.m.
Please
ensure that you include a notarized copy of your NDHCB Certificate and a
notarized copy of your College diploma with your application form. Also,
please ensure that you have your insurance coverage in order prior to
applying. Insurance coverage is available from the ODHA, the CDHA or a
recognized insurance carrier. Incomplete applications will cause
delays, and you may not get a seat.
The
$500.00 Clinical Competency application fee is payable either by cheque,
money order, Visa or MasterCard.
If you have previously
attended a Clinical Competency Evaluation
and were unsuccessful, and we have your
notarized documents on file already, you may
fax in your application, if you are paying by credit card.
